I think there are plenty of internet users not yet savvy to what is paid and what is organic listings. But it doesn't take long to point out and educate to others the difference.
Google does make it easy to make your sponsored ad blend in with the color scheme of a content provider's website, so it's possible that many users might not realize the ads are from Google unless they specifically are doing a query search using Google to look up something.
All ads are can be very effective and they're the driving market place for Yahoo, Google and MSN. A significant portion of their income comes from their PPC and advertising programs. But in this troubled market, even advertisers are hurting some.

Users continue to click on paid search links because they match their search. If your site is not in top 10 search results, then paid search links work to drive traffic.
Google ads(content network ads) do not work the same because they need to engage the user versus direct response ads use in search results.
