If you have a database, then do a questionnaire and have them fill out. When it is returned, they earned a voucher to spend on your products/store.
If you are a retail operation, use a mystery shopper to conduct the survey. Your staff gets nervous watching their own interview customers and on that given day, they are on best behaviour - which means you don't see the problem. Most research firms are able to do this for you and it isn't very expensive. |

What sort of problems are you referring to:
1) problems that they have with your products and services
2) other problems they have that as a business you may be in a position to solve?
If (1) then the type of things you can do include:
a) Ask them on a one to one basis
b) Run focus groups and at each one focus on a narrow range of services/ problems they may experience
c) Survey them
d) include them in a holistic business review (http://www.rapidbi.com/bir/index.html )
If (2) then either survey them, or undertake a range of focus group sessions focused on other business issues they have they could be solved by you - or things they think you as a business may be able to help with.
Without knowing what sector you are in, or the nature of the 'customer problems' you are thinking about, I cannot give more specific information

Provide the incentive as a coupon or buy one get one sort of idea for helping to make the company better.
