If they work for a newspaper/radio/web/etc - they sell space/time/banners, etc...
I worked at a newspaper for years and we NEVER took the graphics designer to the client. We'd take an idea back to the office - get it pretty and take it back out to the client. It was the reps job to sell it... the desingers job to take an idea and make it look good.

It depends on the medium, company policy and there expertise. My online company allows my reps to wear many hats. I train them to develop ads and most importantly good headlines, they can create copy, make sure of the clients objectives, and consult as to what kind of meduim is best to run there ads. Graphic designers can come in at anytime for print or online promotions they have to be a part of the creative process.
