Take your ID and birth certificate and go to the SSI office. Had to do this for my daughte rencently. If you do not have a state ID and your a minor guess your parent going with you and they have this ID and you have your bc you should be fine. They give you a temp paper and mail it to you. |

I think you have to bring legal documents like your birth certificate and I.D.
