What you are asking about is a very rare birth defect.
They both have to eat if they both have a digestive system, but usually they don't. When that happens usually one head depends on the other for nourishment. You may remember the recent case that was so publicized where one baby died when they separated them because she didn't have all the necessary organs to survive, but if they hadn't separated them both would have died.
