Yes you receive what they send! Paypal has an automatic currency converter so you don't have to worry. When they send money it automatically converts without having to do anything!

The buyer would have to pay the USD amount displayed on their page. Someone in Japan would see that it costs 110 USD (293837 yen) etc. You would get the amount you see/set it at.
