Of course he would be consulted, after all you are directly responsible to him and he may not want to lose you (or he may be glad to see the back of you).

Your manager would be notified and would have to have input. Your employer can, and will, turn down your request for voluntary redundancy if they consider you to be too valuable to lose. Only your manager can advise your employer as to whether or not that is the case.

It is very likely your manager will be consulted - after all, only he can really tell personnel how valuable your role is (or not).

Before applying for anything, always know the ramifications. For instance, if you work someplace where they have online tests, and you just want to see what the test looks like, realize others will be able to find out you took the test. :D Or in your case, you basically did something a manager will know about...and you should ask personnel, or human resources, before doing such a thing. Now you can go find out from them since you already did it.
