While I agree with Crystal, keep in mind that Delaware charges a franchise tax. For LLC, usually it has to be paid by June 1st and it is 200. If it is not paid by then, it is a $100 late fee plus interest. Also, if you form in DE, you will not be recognized by the state you are operating business in as a business entity. This means that you would have to get a Certificate of Good Standing from DE and then Foreign Qualify into the state you are operating in. This is something that may be a concern for example when opening a bank account for the company.. Some banks do require paper work filed with the state you are operating in but that is something you will have to talk to your bank about.
Also, something to keep in mind is that in virtually every state you need to have a Registered Agent appointed. This person accepts state mail and Service of Process paperwork. A registered agent must have an address located in the state you are forming in or foreign qualifying in. I would recommend going to BizFilings.com. This website has a lot of useful information. Also, the company can help you form a business, foreign qualify, etc. They also can serve as a registered agent if you need one.

Just like a corporation you have to file articles of organization and file it with the state of your choice. you can get all the instructions from secretary of state websites. the easiest state for doing this, I would say, is Delaware, as they are business friendly.
in general though, you should call an attorney to help you draft the articles and guide you through tax issues and other pertinent laws.

Do it yourself. Every State has this on line. Many even let you fill out the forms on line. Here is a list of each states web site ( http://www.candidity.org/incorporating.htm )
There is no point in paying a service company to file the same forms you can file yourself for free.
There is typically no tax advantages in incorporating in other states. For example even though Nevada is a tax free state your home state will still likely tax you for the revenue you produce in their state. Unless you plan on becoming a big company and have lots of legal assistance to deal with complicated tax codes and such you are likely better off just incorporating in your state.
In addition, if you incorporate in your state you can be your own resident agent and not pay for this service.
