1. Poor hygiene in stores & amoungst staff in food shops
2. Not fulfilling promises and undertakings
3. Being dishonest or misleading through advertising
4. Bad client liason and handling of complaints
5. Poor service standards that don't meet expectations
6. Overpricing then offering huge discounts
7. Badly trained or rude staff.
8. Staff behaviour on and off duty
9. Bad press through any of the above

1) Negative press/journalism
2) Poor customer feedback
3) Unethical/illegal trading
4) Making financial losses
5) Bad debt
6) Adversarial relationships with customers/suppliers
7) Late payment to suppliers
I won't expand on each and every one, but that should give you some ideas.

Anything at all. It all depends on how and when it's reported. A light bulb going out in the basement could damage a reputation if it's reported with a negative spin.
