Comcast, I recently learned in CO Springs, that because of some agreement with comcast there can be no other cable company here, at least for now. Direct tv is available through qwest but that is satellite.

There are also local monopolies that may be geographic such as a rural town with one gas station. In the United States monopolies produce about 5% of total output.
