Do you know if it's on your credit report yet? If it is and it's not a legit bill then you can dispute it and they have 30 days to prove to you that it's legit or it will get taken off of your credit report. If it isn't on your report then you need to call the number on the bill and try to figure out what it's about. If it turns out that it is from some services you had then you're going to need to set up payment arrangements.
Every place is different, but it wouldn't be held as a lien against you but they could file for a judgement against you. The company that I worked for only filed a judgement if it was over $500.

I wouldn't just pay it without a few questions. Ask them to provide you with a list of what the money is for. A simple visit to the Doctor shouldn't cost that much. See what else they are charging you for and why it took them so long to bill you. If it is a fair bill, then work out a payment plan rather than just having to find all that money at once.

call the doctor's office and tell them to check their records because you did not have a blood test. you do have to pay for whatever you did have done though. I would want to know why they are billing you four years later though.

I'm not sure about in America but here in South Africa any claim for debt "expires" after 3 years and a debtor has the right to refuse to pay the debt. Not many people know this though. To be honest I would be very weary of paying this account. I would dispute the authenticity of the details i.e. the blood test and would request proof from the doctors office. Since you say that you did go and sort out your acne, perhaps you can make an offer of 200 dollars in full and final settlement of the account and see if they accept it.

write them a letter requesting information about this bill. Did the doctor take blood from you? If he did you would probly get a bill from the person who reads the test and gives the results. sounds a bit fishy to me you should check into this immediately dont let this bill just sit and go unchecked. it could ruin your credit rating

you do if you want it not to show up on your credit report.
and make sure to get a letter saying that you payed it off and it has a zero balance if you pay it.
