You can do a cash advance by going into your branch and present them your credit card and a valid driver license and they can do a cash advance for you. You can also call your credit card company to send you a new pin but that will take a few days to arrive so for now...you will have to go into the branch and do a cash advance.

Phone your Bank and they will send you a new number in a sealed disguised envelope in a few days. You will not get it from your bank even if you take any form of identity as no one is allowed to know your pin number except for you. |

Probably the card itself and identification. An old receipt from the card would help but no one keeps those. Any Documents about the card.

You might need a passport or driving licence.
You could try with a bunch of utility bills proving your residence, but you'd be better off with something with your name and picture on it - such as a passport or driving licence.
I doubt they'd give you a new pin with anything less - also, lots of banks/credit car co's allow you to change your pin online - as long as you have the credentials to log in.
