With health insurance they do not test at all. They go by the answers you put on the medical questionnaire.

Once a claim is filed the insurance company will request medical records and if your doctor has noted in his file that you are a smoker the insurance company can rescind your policy and you will be left paying for any claims.

Most carriers ask if you have smoked in the past 12 months. Once you successfully quit for 12 months you can have the insurance company adjust your rates to non smoker.

Not usually. But keep in mind that if you lie about being a smoker, then any medical problem that you have as a result of smoking with void your insurance coverage for that problem.

No, they won't test you, but they will ask you about it on the application. If you lie, and they find out, all coverage will be void.

They don't test for anything. Health Insurance Companies compare your application to the results found in the MIB (Medical Information Bureau). The MIB is similar to a credit report on your health. If there is a discrepancy, they could order medical records.

i believe it depends on the insurance carrier. however, nicotine leaves your system in a couple of days, it couldn't hurt to quit for a day or two leading up to your physical.

I doubt it. However, if you choose to tell them you smoke, they probably won't cover certain illnesses that can happen because of smoking. Depends on the insurance company.
