Check with your Chamber of Commerce. Each of the states differ on who carries the Workers Compensation (private insurance company or the state), and how many employees you have working for you before you are required to have Workers Comp. Local Chamber of Commerce can direct you who to contact for these questions.

No. All employers do not need Workers' Compensation. All states except Texas require Workers' Compensation (TX is voluntary; however I would recommend you get it if you are in TX). All states but 5 (ND, WY, OH, WA, WV) have privatized Workers' Compensation markets thus allowing you to choose your insurance company.
Of course I would call your local independent insurance agent and discuss your situation to ensure that the decisions you make regarding insurance are what's best for you and your company.

Only those that are legally obligated to. It depends on which state it is, but usually if you have less than a certain number of employees then you may not. Generally, all companies have to. If you are a contractor or independent rep than you wouldn't have that benefit.

If the employer hires employees in the US, they are required to carry Workers Comp for all employees.
Some states will allow the individual owner of a corporation to "opt out" of workers compensation, but the form must be filed in advance of the claim.

All employers who employ one or more employees are required to carry workman's compensation insurance. Although penalties vary by state, you can be subject to heavy fines (in Id it's $25 per employee per day) civil prosecution, and even criminal charges if an employee is injured.

If you work "on the books" then you must have workers comp. protection. It is usually half your salary, to get you by, but thats still not much. It is wise to have some private accident insurance. It will protect you financially on or off the job . I pay 24.00 a month through my union for 600.00 a month if Im injured. Peace.
