It used to be where when you started a job they gave you lots of forms to sign, and one of them was to accept or reject the company's health insurance. When people are married and the husband works then they have the option of being carried under their husband's insurance (if that's allowed, and if the coverage is better) and can reject the secondary health insurance where they work. So your answer is yes, but only because it's the employee's choice. |

Yes. Some "grandfather" coverage...like they gave 100% coverage back in 1990 for all employees but in 2007 they have found that it is too expensive so all new hires are covered 70%. They can limit coverage by position held, number of hours worked....frankly if they are a privet employee they don't even have to offer insurance at all. |

Sure. There's no law stating what benefits an employer can offer. If they choose to offer some benefits to some employees and others to different ones, it's perfectly legal. |

As long as it isn't illegal or immoral, an employer can do pretty much what they want. |

I don't see why not... Especially if the positions held by the employees are different, or they pick different plans. Health insurance is a benefit, its not mandatory. |

Yes. The employer has to make health insurance (if offered) available to all eligible employees. Whether or not they pay for it in part or in full is another ball of wax. |
