Yes, they need to determine whether your current inability to work is based on an injury you incurred while on your current job. If you don't supply them the information needed to make that determination, they can deny the claim. I'm not saying that it has occurred in your case, but Worker's Compensation insurance fraud is huge in this country. Something we all pay for.

Yes, you do. And if you don't cooperate with Workers Comp, don't expect them to stand up for you, or to pay you. My daughter is an investigator, and she would never pay on an account of someone who doesn't cooperate. She would wonder what they are hiding.
Her husband is a Private Investigator for Workers Comp. He takes videos of those who say they are hurt and aren't. Trust me, there are ways they can catch you, without you knowing it. If you truly deserve compensation, you truly will get it.

Work with them, so they can work for you.

If you want them to pay the bills, give them the information.

Yes, you do. At any time any insurer can ask - IN WRITING - for a copy of pertinent medical records before or after they pay a claim. If you refuse to comply with their request, they can refuse to pay... Guess who will then have to shell out? YOU.

yes

Yes - otherwise they can deny your claim.

Yes, or how would they determine how your injury affected your ability to do your job?

Although normally the doctor writes a report for them.
