It depends on the insurance company and the exclusions in the policy.
There have been a few court cases where people killed while DUI have had to sue to force the life insurance company to pay some have won some have lost.

These are the common exclusions.

Suicide Clause: The suicide clause usually states that the company will not pay the face amount of the policy if the policyholder commits suicide within two years of purchase. In these cases, the only payment is a refund of premiums paid. This is to prevent someone from purchasing a policy and committing suicide with the intent of clearing their debts and removing themselves from a difficult financial situation.

Aviation Exclusion: The aviation exclusion usually states that the policy will not pay if the policyholder dies in a private plane crash and not as a passenger on a commercial flight.

War exclusion states that the policy will not pay in the event that the policyholder’s death occurs as the result of a war.

Dangerous Activity Exclusion: The dangerous activity exclusion states that there will be no payment if the policyholder dies while participating in a certain activity, such as hang-gliding, rock-climbing, or auto racing. These exclusions are a little more rare, however, as most companies will cover you at a higher rate if you regularly participate in these types of activities.

Even though 9-11 could have been considered an act of war all the life insurance companies paid up to avoid any kind of bad publicity.

Unless there is a clause against it, it should not be an issue.

In regards to 9/11, I distinctly remember several people on the news and in papers that have still yet to receive insurance payouts even 2-3 years afterwards. They couldn't produce the body to prove they were dead.

There would be a beneficiary named on the policy. Unless the policy has a stipulation about drugs and/or alcohol, they would receive the money.

Life insurance isn't like a check, it's not "cashed". The claim is filed, the death is investigated, and if it's legit, they pay out.

If the policy is contestable, and you lied on the application about the drugs/alcohol use, the policy isn't going to pay out.

Depends on the policy but normally no. endangering yourself normally mitigates life insurance. It could also depend on the drugs and alcohol levels, but unless this was a pure accident it may be hard to recover.
