In order to give you an accurate answer it would be helpful to know the circumstances of your injury. Put very simply, State Farm is telling you that their insured is not responsible for your injury and is therefore denying any liability.
If you are certain that their insured is, in fact, liable you need to contact an attorney who agrees with your assessment and is willing to file suit.

Coverage varies from policy to policy. There is no one answer. If you were hurt on another party's property and feel that they should be liable, you will have to sue the other party directly. You cannot force another party's insurer to pay.

Liability coverage means, the homeowner was negligent - they CAUSED the loss. If they shot your ear off by accident, Dick Cheney style, there should be coverage.
But there isn't automatic coverage, just because it happened at their house. It would wildly depend on how it happened.

I'll assume that this was on someone else's property and not your own. You did not tell us how the accident happened, so no one here can give you an opinion on who is liable, however if you feel that the property owner is liable then file a law suit. As for coverage, the company will pay up to the liability limits for any claim the property owner is liable for.

Your property or someone else's property? If it was someone else's property, it depends on how it happened whether or not their Liability Insurance will cover it. If it was on your property, that's what you are supposed to have medical insurance for.
