There is no such animal. If it's employer based, you should have also received a card. Check with your employer or if it's an individual policy, who do you write the checks out to every month?

There is no website for that. If you are talking about employer-sponsored group insurance, go to your HR dept or your boss.
If the insurance is private coverage, you should have a certificate or policy for each one. If it's for health insurance you should also have an insurance card.
If you have misplaced the policies or cards, who sends you billing statements? Where do you send your payments?
For health insurance, if you have any medical or prescription bills, you could go to the provider and ask them what company they billed for payment.
There's no other way that I know of to find out what company or companies you have your insurance with.

No such horse. No one collects that information, it's private.
You'll have to look at your checkbook to see what company you pay every month, or ask your employer.

Where do you send in your premiums to that is a great place to start. But I would also check with the MIB (medical information bureau) if you are looking for anything related to life product insurance. They do charge a fee but they can help people find lost policies. I know a couple of people that this has worked well for. Good Luck.
