I am almost positive that they are legally required to offer some form of mental health or chemical dependency related care.. but the degree of coverage is very non-specific. Of course, they typically go with what is cheaper for them despite what might be best for you.
From personal experience, I know my insurance covers limited inpatient services.. but they have no problem with extended outpatient care.
They all have websites and numbers for reference on the back of the card.. and they all have different standards of care. This is a question that only you can answer. We can only guess. |

Depends on your plan. |

If your health insurance doesn't, then your employer probably does. |

Most do. There is usually a special 1-800 number on the back of your card for mental health referrals. If not, check the carrier's website. Good luck! |

Some plans do, some plans don't. Some plans cover it on an inpatient basis, many cover it on an outpatient basis only. The only way to know for sure is to call member services (the number is on your card) and ask about what is specific to your plan. |
