They won't cover it solely due to mental anguish, or isolation. The only time I've ever seen it covered is with an grade II or > diagnosis of gynecomastia. Pain has to be significant, hormonal problems have to be ruled out. Go to your doctor.

get your doctor to certify that the surgery is necessary from a medical standpoint due to health reasons, If he won't then it is elective surgery and shouldnt be covered. |

You need at least one doctor (preferably your PCP - the one you see most often) to provide documentation of medical necessity. From there, the insurance company decides whether or not they want to pay for it. If it's not a covered benefit, you can't force them. |

the insurance company will ask you what doctors you've seen and what their opinions are and will probably want you to see a doctor that they specify
