Very confusing story.
I'm 16, I'm not living at home as I was kicked out a few months ago..
I lived with my dad until I was 11 with the last name thats on my birth certificate.. I moved with my mum and she put me in school with her last name, but I didn't think anything of it, I don't know if she changed my name legally or just put me in school as that because she wanted me to take her name.
BUT
My National Insurance came through with my mums surname, and not my birth name... Does this mean my name was changed by deep poll? I'm not sure of how they get your details thats all.
My mum used the name on my National Insurance in school and for my bank account, but I changed my bank account name back to the name on my birth certificate.. as thats what I'd like to be called as that is who I am.
If my name has been changed, how do I change it back? And how do I get in touch with the National Insurance to change the name on my card?

You mean DEED poll.
In the UK, you can call yourself whatever you want, as long as you're not doing it to commit fraud (such as by pretending to be someone else) - but the best way to ensure that you don't have any problems is to change your name back by deed poll. This is when you sign a document saying that from now on you wish to be known by a particular name. To give this a legal standing you need to do this in front of a solicitor. The best way to find a suitable one will be to ask at your local Citizens' Advice Bureau - look in the phone book for their contact details.
Once you've done this, you will need to contact your local Job Centre Plus (again, look in the phone book) to get the form telling them of your change of name - if you have a passport, you will also need to inform the UK Passport Agency.
It sounds as if there's a bit of friction going in between you and your mum over this: is there anyone, such as a teacher or school counsellor, that you could talk to about this? |

I think I might be able to answer some of the confusion here.
Do you know for certain whether your parents were ever married or not?
If your parents are not married when you're born, then your LEGAL surname is your mother's surname (regardless of what is written on the birth certificate).
So I'm wondering if perhaps your parents weren't ever married and therefore your mum's name is your correct name, so even though you'd been known as something else previously she didn't actually change your name for you.
As others have said, if you want to use your dad's surname then you will need to change your name by deed poll to his name. I would advise that you do this as now that you're getting older and will be applying for jobs, college, etc, it will become confusing if people are using two different names.
When you received your NI card, it should have come with a letter stating who to contact if there were problems. If there is a return address on the letter, then write to this address. Otherwise, I would contact your JobCentre and ask who to get in touch with, as NI numbers are dealt with different organisations in different areas. |

Get in touch with the Department of works and pensions and explain about your name change. It doesn't cost anything to change your name, as long as you let every one who needs to know about your name change. Your boss, your bank, your insurers, your passport. etc. |

it should be free to change your name with the NI as my boyfriend has just changed his by deed poll with all of our companies! It was such a pain. You will need to go through everything that has your name on and probably change it so it all matches.
I would contact NI first and see what they advise but all you should need to do is send them a letter explaining what has happened and a photocopy of your birthcertificate. |

I would have a word with your mum, she has no right changing your name with out your consent. Your best asking her if shes changed it on your birth certificate. or you could ask a family member see if they know. I have no idea if you can just change it without proof of your birth certificate so depends which name is on there. good luck |

You can change your name at your local registary office and if you cant do it there then they will tell you where.
I know a new birth certificate costs £15 but Im not sure how much it is to change your name.
Not too sure about NI - google them for a website and contact details :D |
