Hi,
First - try thinking and searching where and how it got lost or misplaced
second - if the first doesn't work, then try asking the insurance agent you bought the policy from.
Third - if the first and second are both futile, then you can go to the insurance's branch office (any branch) and request for a copy. Pls provide the relevant documents, such as an identity card or maybe the policy holder's social security number.
Fourth - if the branch office says that they can't give you a duplicate of your policy,then maybe you should head to the HQ.
Fifth - if they still are not giving you your duplicate policy, make noise, threaten to send letters to the media,etc.
Sixth - try not to lose the duplicate.

You should be able to get the info as long as you have the name of the company. You will need the person insured name and dob. |

Are you trying to collect a claim? Has the policy matured?
If not, then you will probably get a notice in the mail that a premium payment is due, and you will have the info you need on the statement. |

Contact the company -- even online. They will need a statement saying it is lost or destroyed -- but getting a new policy is very easy. |
