Contact your State Insurance Department. Insurance Commissioner web sites usually have a form on the web or one that you can print out to file a complaint. You will be able to find your State Commissioner web site through the National Association of Insurance Commissioners (just click on your state):

Depending on the insurance they will have different procedures for this. Medicaid and Medicare have numbers especially for these concerns also Medicare gives a reward for each case that is reported. You can call most commercial insurances and be directed to the correct area that handles fraud.
