I assume that you are going to be covered under the health insurance because you will be listed as his "common-law" wife or partner. If that is the case the letter will have to say something along the lines of ....
I, (insert boyfriend's name) hereby certify that (insert your name) currently resides with me at (insert your address) as my common-law (use the term wife or partner depending on which way they want it). She has lived with me since (insert the date) and I consider our relationship to be of a permanent nature and hereby request that she be added to my health insurance. If you need further information, please contact us at (insert your phone number at home). Thank you for your assistance.

You need to draft the letter (that states that you cohabitate) and have a couple of blanks for you both to sign. And a blank for the Notary to notarize and sign. Take this letter (unsigned) to a Notary (you can find them at banks and mailboxes outposts, they are easy to find). The Notary will witness that you both sign the document. They will check your picture IDs to prove that you are who you purport to be. They will then notarize your signatures (stamp with an official stamp and sign and date this) and, they will likely charge you a fee (about $10). It's really a simple thing. Good luck! |

He is asking you to get it notarised. You need to go to a notary to do this. Banks always have a notary in them. It usually costs $10.00 to get something notarised. When it is notarised then the letter is official and legal. |

Notarized just means that you sign the document in front of a person called a Notary. They just validate that it's your signatures on the letter. They usually stamp and sign the document. |

You write the letter, and bring it to a Notary Public. Someone who is certified to "watch" you both sign the letter, after he whitenesses the signing, he stamps his seal into the letter and signs his name to it. Now your letter is notarized. Goo dluck. |

all this means is to have you and him both sign this statement in front of a notary public. you cant just sign it in from of each other. have him create the statement, look in your local phone book or go to www.411.com or www.google.com or www.superpages.com and look up a notary public in your area. teh charge is usually about $20-$40 for signatures. and when teh notary arrives, he or she will then have you both sign on his or her presence. she or he will then stamp your statement with teir individualized notary stamp and then he can take your notarized statement to his work. |

You don't write a notarised letter. You need to see a notary (they usually work with a solicitor). The notary will then write the letter for you and then, you will be required to go and see another notary and get the letter signed and authenticated. It's easier than you think. I know you will also have to pay a fee, but, I don't know how much that is these days. I remember having to get some authorised letters in he 80's for my divorce and they were 15.00 (Sterling) per letter. |
