I would call all of the above! Call the collections company to let them know this is being disputed. Call the doctors office to tell them they should not be billing you for this and to get a copy of the full itemized bill. Then call the insurance company and fax them the bill and tell them to take care of it for you!

you should first call your insurance company and ask them why its not covered, then call the collections company and explain to them what your insurance said. If your insurance ends up not covering it then you need to work out some type of payment arrangement with the collections company, if not it'll ruin your credit. |

your insurance company...they will send you a copy of the bill but you call your insurance and tell them that you keep getting a bill |

You need to dispute it with your insurer. But in the meantime, the provider has evidently sent the debt to a collection agency so its now a black mark on your credit record. From the provider's point of view, you are responsible for whatever your insurance doesn't pay (for whatever reason).
Pay it now, then try to get the insurer to cover it. |

Well, the actual BILL is correct - you got the service. So you can't dispute the BILL.
You can check with your insurance company to see why they didn't pay it, and fight it from there. If you can't get them to pay it, you're going to have to pay the bill. |
