Health insurance premiums are calculated by determining the claims experience of your group and then modeling that and other age and demographic factors into a rate that is expected to cover the claims liability for a given plan year. Although Group Plans are designed to help absorb the cost of individuals with higher risks/expenditures, it is erroneous to state that there is no effect on the rates. In fact, to the contrary, since statistically smokers will increase your overall plan costs, they will therefore increase your claims experience and inflate the cost needed to cover that liability.

In other words - higher rates. Offering a credit or discounted rate to non-smokers; or for smokers enrolling in an accredited smoking cessation program, are both ways to help reduce the risk of cost to your plan. This, and other like Preventive Care measures, are your best way of controlling plan costs, regardless of the size of your insured population.

I am in a large company (19,000) employees and am a smoker. I pay a higher rate than a non smoker. I know people sometimes think this is wrong but if you think about it a smoker has more of a chance for constant respiratory ailments.

I do believe a lot of the insurance companies now days offer different rates.

When you offer the benefit, you should pay a flat rate for each employee. Then, if the employee has a higher premium, they will pay the difference, and if they refuse the coverage, you can give them a bonus in the amount.

That I don't know, but a lot of smaller companies have the smokers pay a bit more in the premium department to defray the extra costs.

As long as it is group insurance, it should not effect the price that much. That is why group insurance is offered. To help out people like that. The bad side to that is, those employees that do not smoke, and are much healthier will possibly pay higher premiums versus getting individual insurance.

Depending on the size of the group, it may actually be more "individually underwritten", if it's a really, really small group. If it's a really LARGE group, then the smokers won't affect the premium at all.

Unfortunately, "small" and "large" are defined by each insurance company, so the agent that writes the rest of your business insurance, who can give you quotes for group health insurance, can tell you which way it would be for you.

Blue Cross Blue Shield of Arizona does not ask if an employee smokes or not, and this may be the case in your state.
The reality is that smoking may cause several medical conditions, conditions an insuror will certainly ask about, and charge accordingly.
