Depending on what state you live in you may not be accepted because of all of your health problems. If you are accepted you will likely pay very high premiums or all or some of your pre-existing conditions will be excluded. The general rule with individual health insurance is that the you must get a clean bill of health for each pre-existing and be treatment free for one year before the coverage will apply to said condition.
If you get a job that offers health insurance than they must offer it to you. There still may be a waiting period for treatment, but it would not require you to get a clean bill of health first.
Feel free to contact me for a list of the states that require health insurance be made available to all residents.

Yes, well, ANY ONE THAT WILL TAKE YOU. I don't know of any insurance company that's going to offer you a policy with heart problems.

You definately need more help. I'd strongly suggest doing this offline, and going to a local, independent agent, and seeing what they can scramble up for you. It doesn't cost you any more money, and you've got an expert to help you out.

The following comments regard Individual/Family policies, not group.
You have mentioned your family medical history, but from your statement it appears that YOU are currently being treated for hypothyroidism and anxiety. If this is the case, it is these two conditions that will be underwritten.
Hypothyroidism is usually not a concern for an insuror, the anxiety is a little trickier. If you are taking more than one medication for anxiety, this may cause an exclusion, each company differs.
Keep in mind that you must also qualify by height/weight, generally no more than 40% overweight.
Apply online with your local Blue Cross Blue Shield, taking a high deductible to keep your cost down. Medications will be covered most likely without having to meet the deductible first. Hypo meds are very inexpensive in any case.
