From a banking perspective, checks are "stale dated" (expired) after six months - unless stated otherwise. (and it would say it clearly - often near the signature line) Even though you technically have a month, I'd run to the bank ASAP if I were you.

Probably. It is from less than six months ago and they usually do not check. If you have a problem, you can always have the health insurance company stop payment and issue a new one. They are not allowed to keep the money.

Checks are only good for six months. But you can ask the issuer to re-issue the check for you.

You usually have 9 to 12 months to cash it. I don't believe you should have any problems.
