Your first step is finding out why the claim initially denied. (Which is what you should have done in the first place, prior to writing out a check for the service. Always verify that the claim denied correctly before paying the bill yourself.)
The way you'll need to proceed depends upon why it denied. Did you use an out-of-network provider? Did the provider bill incorrectly? Did the procedure need pre-authorization and you failed to obtain it?
If it was an issue with the way the claim was billed, you need to make sure that a correct claim is billed to the insurance company.
If it was an issue of not demonstrating medical necessity (not obtaining preauthorization, etc.), then you need to have medical records sent for review.
Also, keep in mind that there are generally appeal and claim filing deadlines, so make sure that you stay within those time frames.

You will first have to contact them, in writing, with all details. You have to give them the chance to make it right. If they refuse, see if there is a grievance process. If so, follow that. If the grievance fails, or there is no process, you'll have to sue them. |

I would suggest the follwing:
1. Call the carrier and advise the reasoning for the claim denial. (Also, obtain the time-frame on the appeal policies- will vary by state)
2. Obtain the information necessary to contest the denial. (usually requires the partnership of the doctor, overall suggest to obtain all the doctor records for the procedure)
3. File an appeal directly with the carier.
4. If carrier upholds the denial explore remedies with the HR department if you feel comfortable. If not, go directly for the department of insurance.
The key is identifying the root cause of the denial. A claim is rarely just denied by mistake. Once the root cause is determined you must fill the information gap to trigger the payment.
