Who do I contact to file a formal complaint against a insurance company in the state of Illinois? Does this require a specific form? If so how would I go about getting one?
Most states have an Insurance Commissioner that regulates the insurance industry in that state. Look up info on the Illinois State Government to locate the State Insurance Commissioner or Department. It is likely that they have a "user friendly" consumer complaint process, a help line, website, etc.

You should NOT complain to the Illinois Insurance Commissioner, unless you live there.
You should file the complaint with the Insurance Commissioner of YOUR state. That commissioner regulates insurance in your state...which your policy is under.

As above, it'd be the DOI of Illinois...But is it possible to sort this out with your insurance company first? Possibly directly contact the examiner in charge of your claim and see if s/he can help you out? Often, the examiner is very willing to help, even if the customer service department is...rather less than helpful. (Can you tell I worked on 2 unnecessary DOI complaints today? *grin*)
