Coverage will depend upon the actual plan and state. If you have a Blue Preferred or BC group plan, you will have to pay the Doctor Office Visit Co-pay, then the blood panel should be covered @ 80%-100% if you use an In-Network Provider.

You pay for the deductible/copay on the lab test. There is no way anyone here can tell you exactly how much that is.
