Is it legal for an employer require its employees to either sign up for the group insurance, or provide...?
proof of coverage elsewhere? We are in Michigan, and the co-workers are all up in arms about this. Our employer is saying that since they provide health and welfare in addition to our wages, and this is pre-tax, then they can require us to have medical insurance. Their medical insurance is very expensive ($ 400 a month for a single, and that's the cheap plan). They also offer a-la-cart benefits, (vison, dental, 401K, etc), and if you don't use the H & W for their benefits, it is taxed at the normal rate and added to your paycheck. Basically, we are upset because we should just be able to decline (wise, or unwise) without having to prove we have insurance with someone else. Some people just don't have enough for insurance with the company or elsewhere. Those people are being told that they will be signed up without their consent and will have the premiums taken out of their benefit allotment. How can this be legal?

This is likely the employers insurance company, or State law. I have been looking into offering group insurance to my employees in NJ, and have been advised by every insurance group that all full time employees must be accounted for, either by providing them insurance through their group or showing that they are insured in some other way.

I think Insurance Pickle is right on this one.
You might call your Labor Board in your state or Dept. of Labor.

Usually you can decline the health insurance coverage but they have to offer it to you.

They can't require you to take it, but they can require you to sign a form to waive the coverage.

The issue is if they have 100 employees and 40 just don't want it and DON'T have coverage elsewhere the other 60 won't be able to get coverage.

$400 is expensive for one person. Your employer needs to either contribute to the plan to make it more attractive or just drop the idea of having one.
