You need to check your cancelled checks, to see who you paid, and get policy numbers off of them. I'm not sure who's copies were destroyed, but there SHOULD be three copies of each policy - one with you, one with your agent, and one with the company.
There is no central database where you can go look up information, you'd have to get it from one of these places. |

Insurance companies spend millions on computer networks to ensure that their records are correct. They can search for policies by name, address, policy number, or any number of other identifiers. They aren't in the habit of 'losing' policies to avoid payments. (The company risks severe judgments if they get caught doing this crap.)
The first place to start would be your agent, if you went through an agent. They would have the information. Next, try your bank. Since it is very unlikely that you paid in cash, your bank or credit card company should have a record of who was paid. Once you know who received the money, you will know who to place a claim with. |

first check with your bank and have them do a search for amount to match the business name
or the credit card

If you have any canceled checks that you paid your insurance policies with you should be able to summit that to the company as poof. Hoping you put the policies numbers on the check. |

Are you talking about the application or the actual policy? It sounds to me like you mean the application. Applications can get lost in the mail. However, your agent should also have a copy of the application.
Insurance companies are not in the habit of losing policies. If they received the application and sent you a policy they will have records of it in several places and will have no problem finding the information. |
