What can be done about the rising costs of healthcare premiums in the U.S.?
The fact is that healthcare costs rise faster than inflation to the point that more and more people are going without health insurance.

What can be done about this, short of going to nationalized healthcare?

Clearly as the supply of funds from the insured dwindles, market pressure will come to bear on the Health Care industry.

The fastest way to lower health care cost is to restrict the supply of money flowing into its coffers. Health insurance is the fattest calf, slaughter it and see how quickly market forces will correct this inequity. A simple matter of the law of supply and demand.

A more realistic approach is to buy only catastrophic protection health insurance with a very high deductible, say 25,000.00. For most healthy individuals they would probably come out ahead in the long term.

Recently Congress authorized the establishment of Health Savings Accounts that are funded with pre-tax dollars. This option allows the individual or family to put away pre-tax dollars ear-marked for Health Care cost. These accounts are not the 'use it or loose it ' plans previously authorized and can accrue with interest until retirement. At that time theses funds can be diverted from Health Care cost only to partially fund retirement, fully taxable upon withdrawal.

Prevention of health problems e.g tackling the problem of obesity or being overweight. Majority of health problems in the USA is related to obesity and billions of dollars of cash is spent treating overweight patients. For this reason, covering health care costs is proving to be expensive. Insurance companies also want to make their profits so, premiums increase so that the insurers not only cover/treat the ill, but so that they can be able to make profits from their businesses.

NB: some of illnesses/conditions related directly or indirectly to being overweight are: diabetes, heart disease, cancer, sleep apnea etc. I'm no doctor.

It's really simple.....pay more....use less.

If we all paid more in premiums and used the system less...costs would stabilize somewhat.

But the real problem is that we all want Rolls Royce coverage, but we only want to pay for a Yugo.

Once people realize that high quality plans are going to be expensive....then they'll have less expectation of getting cheap coverage.

We need to put more information in the hands of consumers...if you take away the veil of the third party payment system and have openness and singularity in pricing (in other words cash payers pay the same as the big health plans that negotiate discounts).....then people will learn the true cost of their care.

Now factor in the idea that they are spending their OWN money on health care (like the HSA model) and people will be a LOT more careful how and what they spend their dollars on.

An ounce of prevention is worth a pound of cure, or something like that. Spending billions of dollars of McDonald's and then spending hundreds of billions on diabetes and other obesity related care seems ridiculous, but Americans see no problem with it. Couple that mentality with the mindset that people should have access to any and all procedures/drugs, regardless of cost/benefit ratios and you have premiums that have nowhere to go but up.

My solution would be to have healthcare premiums go up in cost exponentially with a persons weight: call it a "weighting factor". If the inability to fit into standard size seats, clothing, etc. won't get someone to stop eating, paying 50x more for insurance will.

Limiting malpractice suits will help too, but nowhere near the extent that having 0% obesity will. Healthy people don't need lots of care, which directly translates into reduced costs and it also directly contributes because there are fewer mistakes yielding fewer lawsuits.

Well, you have to address the REASONS why healthcare costs are increasing, and the reasons behind those reasons.

Here are some of the reasons why healthcare costs are increasing:

1. America is becoming obese. 30% of Americans are obese, which creates increased health care maintenance costs for things such as heart disease, type II diabetes, high blood pressure, and other circulation and general health issues.

2. Americans tend to have poor health habits. 20% of us still smoke, and 85% of us don't excercise regularly. I don't even know the statistics on eating habits. And we expect modern medicine to "fix" all the problems created by our bad habits.

3. We have cutting edge technology - and diagnostic tests and equipment COST A LOT OF MONEY. We insist on having the best, and we want the insurance companies to pay for it. Well, where do you think THEY get their money from?? Equipment is usually only good for 3-5 years, then it becomes outdated - and practically worthless.

4. Medical malpractice insurance rates are climbing through the roof. It's not that we have more bad doctors, it's that we have more frivelous lawsuits. And yes, the malpractice insurance costs get passed along to the consumer - or the party paying the consumer's bills - the insurance companies. And THEY pass along those costs to the insureds. **My solution for THIS problem, is to make any "punative damages" settlements immune to contingency collections from attorneys; and to make any class action suits pay the attorneys ONLY "reasonable & customary" hourly salary.

There, I gave you a solution for #4, now YOU suggest something for the other three!!
