Think of it this way: Do you have food insurance? No, so you pay for your food. If you can't afford to eat you will get charity. Health care is the same. You consume it and pay for it if you can. The difference is that charities have the option of not feeding you. Hospitals are mandated by law to provide care to the indigent.
If you can afford to pay then it is your legal and moral duty to pay. If not, you are provided hospital care free of charge.

The urgency of your condition should drive where you go for treatment. Obviously, something that is life threatening or severe should be treated at an ER or hospital. If you qualify based on the guidelines set by the hospital, you may have your bill written off. This may or may not include the doctors, so you will need to ask.
You may also qualify for medicaid in your state.
Illnesses and care that is less urgent can be treated in county health care clinics and hospitals that have sliding scales based on your ability to pay.
Or you could get a job that offers health care.
