The bills would go to your estate. If there is no beneficiary appointed to your estate, the government will typically auction off all your assets (all properties, all vehicles, all belongings, etc). All proceeds will be taxed, then what ever is left goes towards the debts.

If the person has absolutely no assets, the estate claims bankruptcy and the hospital is out the money.

Your estate is responsible for any outstanding debts. If it has insufficient assets to pay them, the debts are uncollectable.

If you have no money or insurance, the Hospital has no one to bill, and they lose money.

Then the hospital puts a lien on your bank accounts and your house, which the state will sell, to get their money.

And if you have no bank accounts or assets of any kind, the hospital gets stuck with no one paying, which is why so many hospitals have shut down in the past ten years.
