You can call the state insurance department (there is no federal insurance governing body), but I'm not sure how much luck she'll have. Her absolute BEST bet, is to go down to her local homeowners insurance agent, and just ask them, to PLEASE call the employer (with her sitting right there for permission) and help her get XYZ straightened out.
Her agent will likely do it as a courtesy, and can make better progress for two reasons: 1. they aren't emotionally caught up in the recent passing of their spouse and 2. They speak insurance talk, and can cut to the chase about what needs to be done.

call the insurance commissioner in her state. start from there and they will direct her on what to do or who is the correct party to call. good luck and im sorry for your loss. |

In addition to contacting the state insurance dpt. also contact the employers HR dpt. they hopefully can get that going in the right direction & possibly tell you what the hold up is. |

You could also go to the government section of your telephone book.
Each state government has an insurance section at the Attorney General's office, although they might call it something different in your state. In NYS you contact the Attorney General's office, and they put you through to the insurance section. Good luck! |

Go to google and type in "department of insurance your state" and it should come up with the regulatory commission for your state. When you get there try to find the link thats says something like "consumer services" and you should be able to find a contact number. |

Contacting the state insurance commissioner is a good plan, but she could also contact her local state congressperson. |
