You should start by calling your state's attorney general's office.
Then you should call your state's insurance department.
It also would be good to copy your pay stubs showing that the premiums were taken out of your paycheck and send them to those officials plus AFLAC.
They might be able to extend your policy if you make up the missed premiums, then go after the employer to pay you back the premiums you paid but they didn't forward along.
And from here on our, you may want to pay AFLAC yourself each month, instead of relying on your employer to do it.
Also....check your Social Security Statement.
Bad employers in financial difficulty don't just fail to pay their employee's disability insurance, they often don't forward the payroll tax, unemployement, workers comp and retirement contributions.
In short, everything you've depended on your employer to pay should be suspect right now. |

The local insurance commissioner or your local labor board would be a good start |

Look in your phone book under your state government. You should be able to find a listing for the Department of Labor or the Department of Workforce Services. Call either of those and tell them what has happened, they will put you in contact with the right person. |

Contact both aflac and your state wage and hour bureau, then let the experts take over as this is a potential fraud case. |

First I would call the Department of Labor and they will definitely help you. I would also call payroll and ask why are they not submitting your money in time to the Insurance company. I would also call the Insurance company as well and find out what they have received so far. The department of Labor is your best bet. Good Luck! |
