Who should pay for the lab fee for my medical examination?
Last October I took an immigration physical examination as a part of the "Green Card" application. The physician asked me,if I would like a general check-up with the blood sample he has. He said the general check-up will be covered by my medical insurance. So I agreed.

I paid for the doctor's service out my own pocket, because immigration test is not covered by any medical insurance. But then, one month later, I received a $1200 dollar bill of lab fee from a bio-lab. They claim that the insurance company did not reply to their filing. So I am now left with a hefty bill. The insurance company is denying the claim because doctor is out of the netwok.

Here is my confusion: I never was informed of the real cost ( it could have been 1 million for what i know ) of the lab fee, and neither the doctor nor the lab confirmed with my insurance company piror to examing the blood sample. Now do I have any choice but paying for the lab fee?

OK, let's take this one step at a time.
1. The lab can only charge you "reasonable & customary fees" meaning that this is how much most labs charge for this service and no they don't have to let you know how much it is ahead of time.
2. You had the service rendered so you are ultimatly responsible.
3. Most insurance companies have something called a hidden provider provision. Meaning that you since you had no choice over this provider, you shouldn't be responsble. This provision works with most insurance but it might not with your, it depends on who you have. You can e-mail me for further details

Ask the provider to refile the claim with the insurance company but chances are if the Doc is out of network it won't fly. Your best option would be to pay the fee, which is not unreasonable (unfortunately), and go with the knowledge that your insurance is your responsibility. It is not the Dr's fault, it is not the insurance companies fault, and it is not the lab's fault. Most insurance customer pay amazing amounts for coverage and have no idea what their benefits mean or where they are covered. Never hesitate to call the number on your card and talk to a person at the insurance company. That's why they are there. Ask what you need to know and become an informed consumer. Better luck in the future with it!

Through insurance companies, the insurer - that is, the company from which you will have your insurance with. THere should be no costs to the person being examined.

This a great example why everyone should always ask "HOW MUCH" before going in for any testing. It amazes me that we shop around for clothing, auto's ET, but when it comes to medical testing we just go in blind. You say the Doctor is out of network, this is going to be a big problem for you as most physicals are not paid at all when you go outside of network. Ask ask ask and ask again prior to getting any future medical exams. I would suggest you try to negotiate with the bio-lab, offer a cash pay off- if it was me I would tell them you will pay 600. cash today. The last thing is letting it go to collections.

I don't know for sure but I would think it would be your responsibility to see if provider is in network.

The main thing here is when you say they told you that the check up was covered by your medical insurance. Actually neither the doctor or the lab . has to check your insurance. It is up to you to go to places covered by your insurance. But then again you have a case if you can prove that they said that you were covered.
