Obviously it would be very difficult to get the public to accept health insurance premiums using age or gender as a rating variable, but why not lifestyle choices such as smoking or obesity? It's not right that someone who smokes and who is 100 pounds overweight pays the same premium that I do. |

I'll take this more from a group plan perspective. They CAN actually charge more for certain lifestyle choices, such as smoking. Some companies have actually fired employees who didn't quit smoking, as this is something employers may discriminate by when considering employment. My company charges an extra $25/mo if you or your spouse is a smoker.
It is harder to vary rates by overweight or fitness level (even if it is for the fat, lazy slob who eats 3 big macs for lunch every day). Because of ERISA (which governs group benefit plans), companies can not discriminate on the basis of pre-existing conditions. Someone may have a hereditary condition or medical condition that makes them overweight or unable to exercise as much (hypothyroid or a bad heart). Because of this, it's extremely difficult to come up with criteria to separate out the people who just don't take care of themselves from the ones who may have some medical reason.
If it makes you feel any better, they will die sooner, meaning that their pension and retiree medical benefits are likely worth less (if the company still has these). |

You must be on a group that has composite rates. If it is an individual plan, factors such as age, zip code, height/weight, tobacco use, and even occupation go into the rating of the policy. |

um, the best I know, everybody's premium is NOT the same. Those with known health issues or risk-factors (obesity, smoking) will be charged more for individual insurance. In group coverage, everyone in the group pays the same premium to reflect the "average" health of the group, b/c everybody is SHARING the risks, though iin some cases people like smokers still pay more. Why share the risks? Because the healthiest people today may be the ones who are ill tomorrow, and vice-versa. |

They aren't, unless you're looking at all the premiums of employees of the same employer - in which case, the insurance company figures out rates for the group - you get the average rate of all the individual rates. |

It usually isn't the same unless your coverage is through your employer. The premiums do vary by age, male to female, current health status, smoking/non-smoking etc. In my state anyway. And it is more expensive for women. |

Well usually the lifestyle choices end up getting factored in once they manifest into an actual health problem.
Not everyone who smokes gets lung cancer....but those that do and try to switch carriers will get put into a high risk category or denied for preexisting condition altogether.
I do think that we will see the day in the very near future, however, where there will be a medical exam, much like in high dollar life insurance policies....that will be used to determine health insurance rates. |

Actually that's not true.
If your company is paying for your insurance coverage then obviously you would be paying the same amount throughout the company. This is due to the fact that your employer cannot legally pry into your health and therefore to make it "fair", everyone pays the same. The idea here is that the premiums will all even out across the board.
If you're obtaining your own insurance then age, gender, your medical history and your predisposition for certain disorders/diseases comes into play. Your premium may be higher or lower depending on all of th above. You may also be subjected to preclusions in your policy where you may not be totally covered (i.e. asthma, pregnancy, cancer, etc.) or not covered for a certain period of time. |
