I would recommend keeping the EOBs (explanation of benefit statements) for as long you want to prove medical history. I generally don't keep them after five years from the DOS (date of service).

I would keep at least the last years worth depending on the insurance purchased. If it is a policy that is growing in value, it is good to keep the annual statements for the past 5 years so you can see the trends, etc. Your agent should also be getting statements annually as well to help you review and answer questions. If it is just a billing statement then, like someone else stated, just keep the end of year statements. Hope this helps.
