It's asking for a name.
Usually the location. Sometimes given by city name or a nickname for the area it's in.
example:
Texas Savings
"Austin Branch"
or Bank of Omaha "Harlem and Irving Park Branch"
if you can't get that info, just give the address in the branch field. They'll probably figure it out |

they are looking for a routing number.
You can find this on your check
It is typically the first 9 numbers that you see on the bottom of your printed check.
If this does not help, call your financial institution. This is something that you may want for future references such as speedy Tax refunds into your account by the IRS and State Revenue Services.
Don't be afraid to ask your employer also. This is new to a lot of people and when you're doing this, it is always easy to ask first.

It might want the name of your bank and it's routing number and your bank account number that you want your deposit put into. You should call your bank to ask them because they can explain it better. |

I've never had any form ask for a bank branch. I think maybe they mean the Routing Number. It's the 9 digit number that comes before your checking account number.
Banks have branches that ARE identified by different numbers, but that is a part of your account number, so there would be no reason for them to need the branch's number. |

One would assume they are referring to either the branch number or physical address (location) |

Usually your bank branch is the address of where you do your business. For instance, Bank of America has many branches around the country and your's would be the one on 555 Main St, in your town or 222 Elm St.etc. There often are many branches in one city.

The branch number is usually located on your cheques as the number before your account number. It usually is a number.
If you don't use cheques you will have to inquire at the branch or they may have it listed online. |

They're most likely looking for the branch ID number, or just the general location of the local branch of your bank. Check with your employer though. |

Its like a tree. The main office for a bank is like the trunk of the tree. Every bank located in other locations are like branchs. Hence the name. |
