Just address your landlord and say you intend to move out on a specific date. As per the lease agreement [if you had a lease], you are providing him/her with 30 (?) days notice. You can say you will advise him/her of your new address, or you can state it in your move-out notice.

It just has to be a couple sentences - just the facts.

Be sure to keep a copy.

It's always helpful, too, to take pictures of the condition you left the place in -- unless you're sure he/she won't give you trouble. (Good to take pictures, moving in, of the new place, too.)

Are you sure you need to write a letter? If you're not comfortable enough with them in person, then just write a simple letter and leave it on their door handle. Using the mailbox without sending it through the mail is illegal. Just specify your intentions in the letter, and you're good to go. Its a contractual business thing, and unless otherwise stipulated, you can exit it at any point.

Dates are important, most leases require a minimum of 30 days written notice, so date your letter and indicate your last day of occupancy. Ask to schedule a move-out inspection, this will help you make sure you get your full deposit back because they will have to tell you at that inspection what they are going to be charging you for, if anything. There is "normal wear and tear" and you should not be charged for that if you fulfilled your lease agreement (are not breaking your lease).

Standard business letter format. If you do not know these then go to the library and look at a secretarial handbook. This will give you some guidance. Be sure to state your legal name as it is on the lease, Their legal name, and the terms of the move out. When, and why (if needed) be sure to give them a forwarding address when they can send any remaining deposit, and where they can contact you, this is an important part of most contracts.

Keep a copy of the letter, send their copy certified and return receipt requested, attache the certifications and the returned receipt to your copy of the letter for future reference.

In most places, there is a form you have to fill out. Check your contract and any other info you have, if no good, try the relevant government department.

you just write a letter! Tell them when your last day is going to be, and if you want one, ask for a landlord reference for furture rentals. Leave them forwarding information in case someone comes by to look for you.

Just make it simple but make sure you give them least 30 days notice or by your lease/rental agreement. They will likely want to make it proper and accordingly to state laws. Landlords hate to lose good tenants so they are strict.

Write a simple letter and forward to them with your rent check or something. Key is to tell them least 30 days in advance so it gives them enough time to find other tenants.
If you have deposit with them.. state it clearly that what day you are moving out and make sure you leave them your new address to send your deposit check to. They have 30 days to follow through with that as well.

CYA! Follow all the suggestions in the previous answers AND make sure you have proof you delivered the letter to your landlord at least 25 or 30 days BEFORE you intend to vacate. Check your rental/lease agreement to see exactly what the requirements are. There should be a part specifically about vacating. The larger the entity you are renting from, the more strict they will be about following their's and the state's rules/laws. A lot of landlords make a buttload of money off people who don't follow the rules when vacating.
