more "modern" apartments will include cable and internet since sometimes they have a fixed deal with the local cable / phone company, though you can usually opt out and got for satellite dish if you want ( though you may find reluctance since the landlord typically gets a percentage ) |

I've never known an apartment that paid for phone, cable, or Internet. Many do pay for water. Gas and electricity are usually not provided.

All of the above either could or could not be included in your rent. It depends on what the landlord is willing to provide. I have lived in some apartments that include nothing and some that include everything.
Edited after OP last statement. All that has to be covered is the basic living space. Everything else is dependent on the terms of thelease. |

Check your lease. Rent doesn't typically cover any utilities that you have listed above. Some utilities it may include are water, trash pick-up, natural gas. So if your heating is from natural gas then that would be included in the rent, but otherwise no. |

again, ask the landlord, make sure you have everything agreed in writing before you move in, also, make sure you know your local laws.
My contract covers water, some local taxes, and internet, but it doesnt cover electric, gas or phone.
Finally, as soon as you move in, examine everything note all damages you can find, including damages to any furniture, and anything which isnt working correctly, inform the landlord/agent of what you find, this way you cannot be held responsible for those damages when you leave. |

It's listed in the lease agreement. But usually the most they cover MIGHT be gas, electricity, water, sewer, trash. But everything is up for grabs, so ask.
Basic cable/net hook-up is actually a possibility. I offer it in my small building because I live there and the units are hooked into my server. |

Ask your landlord / re-read your tenancy agreement.
In North America, generally a lower rent will cover anything from nothing to heating and water. A higher rent may cover everything. But never phone! Phone is never covered, EVER, unless perhaps you live in a dorm or a suite in a family's house.
If you live someplace like Japan, you'll probably be paying for everything, including the purchase AND installation of, er, a fridge, washing machine, a/c unit, etc. |

It will state on your lease agreement what is included and what is responsibility of the tenant. Normally rent covers water and trash and you have to pay electric/gas, cable and phone. Some places even charge you for water and gas even when the apt is not on its own water meter. |

In most states, the landlord must provide water, sewer, trash collection. In some northern states, they might be required to provide heat, especially if the building uses heating oil.
More often, it is local custom and competition, as well as the building construction, which determines what is provided. Some older buildings do not have separate furnaces for each unit or separate electrical wiring, so they might be included, or the building's bill divided among the tenants. Some landlords have wired the building for cable TV as a marketing tool -- but then you cannot get cable internet/cable phone or VoIP.
You always have to ask the landlord about building design issues (central heat, central air, window AC units, etc), which utilities are included, and which utilities you will need to establish an account for. For each utility where you have to set up the account, you have to find out which utility company serves the area.
The most common utilities for which tenants have to set up their own account: phone, internet, cable TV, electric, gas. The first 3 are entirely discretionary, so the tenant decides what to get where, if at all (except, of course, for those few places where a landlord uses cable as a marketing tool).
