Yes. Even if you only worked 1 week, they must provide with a W2 - especially if they made the proper deductions from your pay.

Depending on your earnings, you could qualify to receive a 1099 from them instead of a W-2, but you will need to receive some sort of statement from them for your taxes (and they are required to provide it).
