Like they say, call your employer. They may use a service to which they can give you their phone number.
If you use TurboTax to do your taxes and your employer uses a service that has a 'deal' with them, you can use Turbotax to download your W-2 information into the program.
As a last resort, you can request your w-2 information from the IRS.

If you are unable to contact your employer, or you have personal reasons why you don't want to, the irs is able to send you a copy of your financial information statement, which has all the information of any and all W2's of the tax year in question. The good thing about that statement is, it is a completely accurate record of all wages reported to the IRS from your employer(s). Call 800-829-1040 for more information about ordering your financial information statement.
