You're going to need to go down to the SS office, they can issue you a replacement, but better yet, they can give you paperwork there that has the SS# |

uou could go to the Social Security Administration and they would give you the number. Or maybe they would fill out papers for you to get a replacement card with the same number. |

It may be on file at the hospital but they may not give it to you unless you got to them and prove who you are. If the child is only a few months old call IRS and ask if they have to have it. If that fails send for it at the Bureau of Vital Statisitcs in your state. They are usually pretty fast, I'd do it soon though. Call the courthouse for the number you'll have to pay a few dollars for another one. |

Sorry folk but a birth certificate is not used to issue a replacement card if the child already has a number. You would have to have a shot record that shows the childs name and date of birth and it must be signed or stamped by the doctor or the hospital that issued the record--NO PHOTOCOPIES, a valid passport, or current Medicaid, or and insurance card that shows the childs name and date of birth or your name (the parent) and the childs name on the same insurance card---AGAIN THEY DO NOT USE BIRTH CERTIFICATES AS PROOF OF ID FOR A CHILD!!! |
