puts single and zero on their w-4 - actually married with 4 kids on the tax return, mortgage and high property taxes - itemized deductions, losses on stock sales, maybe has tax free income from municipal bond investments - takes a lot of different things, but it can probably happen

Very easy... They overpaid $10,000... They can overpay $50,000 or $100,000 or more if they want to.... The government will take your money whether its through withholdings or estimate payments.

Your refund is just the amount of money you had withheld minus the amount of tax that you owe.

So if they had a lot withheld, or if they don't owe very much, they get a big refund.

They could have a lot of deductions and a lot of credits that they're entitled to, which would bring down their AGI and reduce the amount of tax they have to pay. It also sounds like they *must* be having way too much withheld from their wages. That's their choice, if they really want to get a huge refund, but with that much money they'd be better off getting it in their paychecks and then depositing it into an interest-bearing account, rather than letting Uncle Sam use it for free for the whole year.
