You need to download form P50 from www.hmrc.gov.uk and send it with your P45 to your tax office. The previous answer who said you would get it back in 5 days is a bit optimistic - think 30 days!

fill out a tax back form, you'll get the money back in April.

Complete the form P50, together with a P91 and P85/P86 if applicable.
Send this together with your P45 to your local tax office with a covering letter.
Make sure you get any tax back that is rightfully yours.
